Whose fault is this?

IF you are a regular customer of OLD TOWN COFFEE SHOP, you will find out the set menu choices are not in the order list.
Normally I wrote it “MANUALLY” on the top of the order list. Past few weeks my colleagues waited for more than 45 minutes also can’t get their meals. There we figured out it was because the waiter did not noticed the set menu i ordered and yet did not place that order at all. So now…. whose fault is this now?
By right, I should have highlighted to the waiter on it, yet I forgot!
Then, isn’t the waiter should check and repeat our order?
Don’t he feel weird, how come 3 of us but only order 1 meal and 1 drink?
I really think this is his fault, however there always no right or wrong. My friends had been starving for extra 55 minutes just because of his mistake, yet what can we do beside keep waiting for the meal?
